Abstract Introduction An estimated 50% to 80% of CVD cases are caused by modifiable and preventable risk factors, which are assessed as part of the NHS Health Check. This mandated public health function is the only programme in the world of its kind, providing a cornerstone for the prevention of other diseases that share common risk factors for CVD such as dementia and some types of cancer. However, currently only 50% of those offered an NHS Health Check are taking it up. PHE have invested in understanding how digital technology could enhance the programme to increase the reach and improve the impact of the NHS Health Check. Method PHE digital and CVD prevention teams are undertaking an agile project using service design and behavioural insights methodologies. We undertook a discovery phase to understand needs of our users and business stakeholders. Our findings identified and prioritised that our key challenge for the next phase (alpha) should be: How might we improve the way end users receive, understand and act on their NHS Health Check results, and the way providers deliver them, to motivate necessary behaviour change?Our alpha research will run between May and October, where we will design prototypes of solutions, test these with our users and iterate designs based on their feedback. Results At the conference, we will present the prototypes and early findings and feedback from users. Conclusion The PHE conference provides the perfect platform to engage local commissioners and providers and gain their support for further development. External funding details N/A
