Abstract Introduction
-Little support was available to hospital staff to safely discharge patients living in cold homes. NICE guidelines recommend the discharge of vulnerable people from health or social care settings should be to a warm home.Method-Warm and Safe Wiltshire (a joint public health initiative between Swindon and Wiltshire Local authorities providing energy advice) has embedded an energy advisor into the Integrated Discharge Service (IDS) at Great Western Hospital in Swindon over the winter of 2018/19 to help patients return to a warm home, lower bills and registrations for other support services such as the Priority Services Register.
Results
-The energy advisor received an honorary contract to enable working at the hospital and integrate with staff and departments. They were able to:Assist discharge nurses with complex discharges with no heating over winterInteract with staff and patients on wards providing information leaflets and allowing staff to refer easily into the Warm and Safe serviceThe IDS benefitted working with the energy advisor by:Utilising the one click referral process, which has made it easier for staff to refer patientsOther hospital departments are now aware of the service through energy advisors attending meeting and presence in the hospitalConclusion-Excellent partnership working between the hospital, councils and energy advice service has been demonstrated resulting in shared knowledge and a desire to continue next winter with the placement of an energy advisor in the IDS and roll out to other hospitals External funding details Part funded by Scottish and Southern Electricity Networks and Wessex Water
