Nicotine intake and psychometric measures in a longitudinal study of smokers switching to e-cigarettes (Smokefreebrain)

Abstract Objectives. SmokeFreeBrain monitors volunteers as they switch from heavy (>10 cigarette/day) smoking to electronic cigarette (EC) only use for 28 days. We will determine if EC use can maintain nicotine intake at pre-switch levels and alleviate symptoms of nicotine withdrawal. Electroencephalography (EEG) will be used to investigate electrical changes to the brain following transition to EC-only use. Smoking status at follow-up will be used to determine smoking cessation efficacy.
Methods:
. Volunteers recruited and consented at St George's University of London were allowed to select their own EC and e-fluid including nicotine concentration. Saliva, urine and questionnaire data (nicotine dependence, withdrawal, anxiety, depression, mood, quality of life and sleep quality) were collected at baseline (days -1 and -2) and on EC trial days 3, 21 and 28 and EEG data were collected at baseline (day-1) and day 21 of EC use. Nicotine, cotinine and anabasine were determined by UHPLC-HESI-HRMS. Compliance was monitored using expired carbon monoxide levels and basic health data (BP, Heart rate) were monitored. Telephone follow-up 1, 6 and 12 was used to track smoking status.
Results:
. Thirty-one volunteers completed the study. Urinary and salivary nicotine, cotinine and expired carbon monoxide decreased significantly following transition to EC. Significant decreases in nicotine dependence, nicotine withdrawal, urges to smoke and strength of urges followed the transition to EC. Comparison of regional spectral power extracted from EEG data pre- and post- transition to EC use has identified significant increased activity in anterior cingulate cortex in volunteers that decreased their nicotine intake. At 1, 6 and 12 months, 88, 69 and 45% of respondents were non-smokersRelevance. Switching to EC significantly reduces nicotine intake but also nicotine dependence and urges to smoke demonstrating utility as a smoking cessation intervention. Funding EU Horizon 2020 Smokefreebrain
