Do domains of deprivation from the Index of Multiple Deprivation predict overweight and obesity amongst primary school children in England?

Abstract Introduction: In England levels of childhood overweight/obesity have increased for numerous reasons, including deprivation that is often measured using the English Index of Multiple Deprivation (IMD). The IMD comprises seven domains ('income', 'employment', 'education, skills and training', 'health deprivation and disability', 'crime', 'barriers to housing and services', 'living environment'). Whether IMD domains are associated with childhood overweight/obesity remains unknown. This study investigated if IMD domains predict overweight/obesity amongst primary schoolchildren in England. Methods: Secondary analyses were conducted using local authority National Child Measurement Programme 2011/12 - 2014/15 data (n=324), IMD 2015 domains including the Income Deprivation Affecting Children Index (IDACI) and 2011 Census data. Univariate analysis investigated socio-demographic factors and IMD domains associated with overweight, obesity and overweight/obesity, with significant predictors (p<0.05) incorporated into multiple linear regression models. Results: After adjusting for confounders, all IMD domains significantly (p<0.001) predicted overweight/obesity and a positive linear relationship was observed in both Reception and Year 6, except 'barriers to housing and services' for all age/weight categories and 'crime' in Reception overweight. This was more evident for obese than overweight children, and Year 6 than Reception children. The strongest predictors of overweight/obesity were 'overall IMD' and 'IDACI', with 'health', 'education' and 'employment' the strongest predictors for certain age and weight categories. Region and ethnicity were small and significant (p<0.05) confounders. Conclusion: This study confirmed causes of overweight/obesity are multifactorial. Although the strongest overall predictors of childhood overweight/obesity should be used as a measure of deprivation, other IMD domains can be considered in childhood overweight/obesity interventions.
