Abstract Active for Health RotherhamBackground:The AfH research project is an innovative approach, increasing physical activity opportunities for people with long term conditions (LTCs). The project was developed from evidence and pilot activity in 2012, which highlighted the role and value of physical activity and key elements required to embed within healthcare pathways (quality assured activity, partnership with healthcare services, support for individuals). Delivery started in November 2015.Method:The project covers seven LTCs (Cardiac, Heart failure, COPD, Cancer, Falls, Stroke, MSK). The programme follows a three step model from rehabilitation, to moving on and keeping active. It includes a Community Buddies scheme and training programme to maximise impact. The research measures activity levels, quality of life (EQ-5D-3L) and healthcare savings, measured at baseline, 3, 6 months and 1 year.Results:999 referral, 648 take up Step 2, 337 continued to Step 3.Self reported physical activity levels were measured using the IPAQ and IPAQ-E. Levels of moderate activity increased from 117.60 minutes (baseline) to 311.43 (3 months) to 326.86 (6 months), meeting the Chief Medical Officer 150 minute moderate activity recommendations.Quality of life scores increased following the 12 week AfH programme, from 62.02 (Baseline) to 72.25 (3 months), a notable difference considering 0 = the worst imaginable health and 100 = best imaginable health.Conclusion:Physical activity has a considerable role in improving the recovery journey of people with long term conditions, particularly when it is focused and condition specific.
