Abstract The Southwark Exercise Referral programme is a physical activity intervention designed to change the behaviour of participants, which will improve their health and help them to manage pre-existing conditions. This is an evidence based programme which aims to reduce the cardiovascular disease (CVD) risk of participants, not only through exercise, but also by looking at other aspects of a healthier lifestyle. As well as supporting those at risk of CVD, this programme has been developed for people who have already had a cardiac event or have a cardiac condition. This programme is funded by Southwark Council as part of the Southwark Free Gym & Swim initiative which encourages residents to become more active. Public Health oversee the multi-agency steering group. It is an integral component of the main leisure contract, running from five Southwark leisure centres, using state of the art facilities. The prevalence of CVD in Southwark is high and the programme aims to reduce health inequalities. There are three levels of support offered, low, medium and high to match peoples' level of risk. The programme includes, gym sessions, Pilates, Tai-Chi, aqua-aerobic and circuit classes along with led walks, healthy living workshops and weight management support. Participants receive motivational prompts via texts messaging to support their attendance.Evidence shows that participation in the programme produces measurable successful health outcomes. The programme continues to investigate methods to broadening the offer and provide greater choice, such as working within local GP surgeries.
